Medical coding is the transcription/transformation of healthcare diagnosis, procedures, medical services, and equipment into an exact mapping to alphanumeric codes related to a specific codeset. It requires skilled professionals to review diagnoses and procedures, physician’s notes, and laboratory and radiologic results found in medical record documentation, and translate them to the appropriate codes.
Speed and accuracy are essential, as records must be coded within a finite period of time within a required accuracy or Acceptable Quality Level (AQL). Personnel need to be certified by either AHIMA or AAPC as a medical coder and have a minimum level of experience to be proficient.
